source monitor and program monitor boxes will not display video in full only half

i have adobe premiere pro cs6 , source monitor , program monitor boxes not display video in full half.  top half of both monitors black , bottom half video displays.  can see video missing top part.  people's heads cut off , can see body.  how can fix premiere pro cs6 use full monitors boxes display video?

this known issue recent windows update.
