Missing image when opening PDF in Photoshop

greetings!

 

i had client send in ad in pdf format. when @ preview on screen images there, when open in photoshop 1 words disappears when try open in cmyk mode (that's how saved). however, when open in rgb mode words there, can convert cmyk. i'm trying figure out why didn't show in cmyk works in rgb mode. i'm running snow leopard (10.6.8) , cs 5. client used id place ad on template. hopefully, have given info needed aid in finding reason this.  image not flatten? that's guess image not flatten, confirm that, or correct me.

check ink separations , object properties in acrobat. either it's tagged non-printing or uses specific color/ ink require converted manually.
