Trouble placing Mp4 in ePub

i trying add multiple mp4s epub designed use in ibooks.

 

i followed tutorial from adobe , seemed pretty straightforward. however, when load epub on ipad , view via ibooks, "play" button on mp4s has slash through them , media not work.

 

some notes: place mp4s , resize boxes better fit page layout, have tried exporting of available epub options in adobe , received same results, , have newest ipad.

 

thank help. searching issue has not lead me solutions.

what version of indesign using? did choose options on media panel mp4s?
